Please print off a copy of the complaint form and send it to the address at the top of the form. Forms must submitted via normal Royal Mail post. You cannot send the complaint form via email.

Before submitting your complaint, you may want to read ‘Submitting a Case to the Ombudsman’, which can be downloaded using the links below.

There are two versions available. One for those of you who have Microsoft word and the other for those of you who have Adobe Acrobat reader. If you have neither visit http://www.adobe.com to download the free acrobat reader software.
